Eyelid Weights and Lower Lid Tightening

What is an eyelid weight?

Many patients with flaccid facial paralysis have difficulty achieving full eye closure; this is referred to as “lagophthalmos.” This puts patients at risk for eye dryness, irritation, infection, and corneal abrasion. Dr. Yver may recommend placing a very thin platinum weight in the upper eyelid in order to help achieve full eye closure. This is typically performed in the procedure clinic, or it may be performed in the operating room when coordinated with other procedures. Unlike the traditional gold weights, platinum weights are thinner, less visible, and have a significantly lower risk of extrusion. If eyelid function subsequently improves, the weight may be easily removed in the clinic.

What is a lower eyelid tightening?

Some patients with flaccid facial paralysis experience sagging of their lower eyelid; this is referred to as “paralytic ectropion.” This affects patients’ ability fully close their eye, and is also aesthetically unappealing. Dr. Yver may recommend a minor procedure to tighten the lower eyelid and reverse the ectropion. This may be performed in the procedure clinic or in the operating room depending on patient preference.

Is this covered by insurance?

Yes, this procedure is covered by insurance for the indication of facial paralysis.

What to expect during these procedures?

On the day of your appointment, you will have a chance to meet with a team member to ask any questions and sign the necessary paperwork. You will be positioned comfortably, and the eyelid will be carefully marked out and numbed with a small lidocaine injection. At the conclusion of the procedure, several dissolving stitches may be placed to promote optimal healing. The entire appointment typically takes less than 1 hour. You should plan to have someone available to drive you home after your procedure.

What to expect after?

Although there is very little discomfort associated with the procedure, you may experience a small amount of bruising or swelling of the eyelid. Patients are encouraged to apply ice for 24-48 hours after the procedure and to sleep with the head propped up on several pillows, in order to minimize swelling. The stitches may take 10-14 days to fully dissolve. We will send you home with antibiotic ointment to apply to the stitches for a week after surgery.

Are platinum eyelid weights MRI compatible?

Yes, platinum eyelid weights are MRI compatible. You will be provided with an implant card with information about the implant. Prior to any MRI, you should notify the technician about the presence of the eyelid weight and provide the implant card, as adjustments to the MRI settings may be required
